Title: Predyktory procesu zdrowienia osób chorych psychicznie w rodzinie

Abstract:

The aim of the doctoral dissertation is to optimize cognitive and empirical activities aimed at achieving an innovative research result, in particular: getting to know the problems of mentally ill people in the family; demonstration of predictors of the recovery process in mental illnesses in the group of inhabitants of the Świętokrzyskie voivodship in the age range of 18-35; developing final preventive programs in the field of mental health. The first chapter of the doctoral dissertation contains the concepts of mental health in terms of Aron Antonowski, Karol Wojtyła, Kazimierz Dąbrowski, Shitija Kapura, which form the basis of views on such values as human health and life. The second chapter of the work deals with the issues of cultural psychiatry, which examines the influence of cultural factors on mental health. This chapter also discusses the state of research on the healing process in the world and in Poland, behavior towards the problem, relationships, attitudes in the family and the environment, and the importance of support in the family with the mentally ill. The third chapter concerns the mentally ill in the process of recovery in the living environment taking into account the need for security, the sense of support and the place of significant people, self-determination, the importance of social and professional roles, positive attitudes in the family, including personal and social support. The fourth chapter of the doctoral dissertation is about strengthening the recovery process in the family, the importance of relatives and friends, medical staff and institutional forms of help. Chapter fourth deals with legal issues as well as the promotion and prevention of mental health in Poland. The work methodology is included in the five chapter, while the sixth chapter presents the predictors of the recovery process of the mentally ill in the family in the light of own research.
Own research confirmed the assumption of the main hypothesis. For 39 respondents, the need for security is the foundation of the proper functioning of a human being and influences every aspect of his life. Acceptance in the family (37 people), positive relationships in the life environment (36 people), self-acceptance (33 people), self-realization (30 people), economic independence are predictors of recovery, according to the research group. Lack of information, disinformation, delay in actions are a source of danger, uncertainty and loneliness, according to the research group they do not help to recover. It turns out that the social status does not improve the sense of security. As further research shows, it also does not affect the recovery process in the family (40people). Therefore, the assumed hypothesis was not confirmed. Acceptance of oneself in every situation (always 36 people) proves, according to the researcher, the maturity of the research group. More than 34 respondents accepted mental illness, which is a dimension that is rarely available in the disability adaptation ladder, especially in mental illness, because it is difficult to accept people who is not acceptable in society, which is the reason for discrimination and stigmatization. Self-determination, being an important link in the family and local community, is extremely important in the recovery process. The declarations of the respondents, especially in the field of self-determination and self-fulfillment, revealed the true face. When analyzing the variables in the field of self-determination, it should be unequivocally stated that the conducted studies confirm a great distance to people with mental disorders. These people feel undervalued, their opportunities for development and work are limited . The mere fact of making decisions, making a choice, has no causative power. Although they are not legally incapacitated, they are socially "invisible". Most of the research group has the status of a disability pensioner (21 people). Only 12 people are professionally active, which the research group. 16 people are unemployed. Data on social status are disturbing. Not working is not good for recovery. The family plays a key role in the life of the mentally ill and provides invaluable support. Family resources are valuable because they constitute the natural living conditions in which this young man grew up and developed. Likewise, the living environment should be the best basis for recovery. As the own research shows, the following factors are of great importance in returning to mental well-being: acceptance of a mentally ill person in the family, regardless of his or her health condition (32), unconditional love (21), undying faith, and hope in possibilities (14). For 37 of respondents, recovery is faster if the family is full (father, mother, siblings).
The sick person must feel that he is needed, important despite the awareness of his own deficits. Under such conditions, recovery will take place. The declarations of the family do not mean anything, they must be followed by specific actions. Another discussed research phenomenon is the burnout of forces, which is closely related to long-term care for a family member with a mental illness. Families and the sick themselves are powerless to face mental illness. Persistent causative powerlessness can lead to helplessness in the fight for the health of a family member. By taking action, we expand the sphere of our influence, then powerlessness becomes wisdom. She gives reflection, teaches us humility and distance to life. Over 17 of the respondents confirmed the assumed hypothesis that helplessness and powerlessness accompany the struggle for the health of the mentally ill in the family. On the other hand, only 14of families feel their strength burned out in connection with caring for the mentally ill. Others (5families) are not affected by the problem or show understanding to the situation. Subsequent studies have shown that 10 of families suffer together sick and over 14 struggling with helplessness. Only of families received pastoral assistance, while for 19 of respondents, no one offered support. The living environment for the mentally ill is a natural environment which, with the participation of the family, therapeutic group and pharmacological therapy, is the right place for mental convalescence and social reintegration.

Celem pracy doktorskiej jest optymalizacja działań poznawczo-empirycznych zmierzająca do osiągnięcia innowacyjnego wyniku badawczego a w szczególności: poznanie problemów osób chorych psychicznie w rodzinie; wykazanie predyktorów procesu zdrowienia w chorobach psychicznych w grupie mieszkańców województwa świętokrzyskiego w przedziale wiekowym pomiędzy 18-35 roku życia; wypracowanie końcowych programów profilaktycznych w zakresie zdrowia psychicznego. Rozdział pierwszy dysertacji doktorskiej zawiera koncepcje zdrowia psychicznego w ujęciu Arona Antonowskyego, Karola Wojtyły, Kazimierza Dąbrowskiego, Shitija Kapura , które stanowią fundament w poglądach dotyczących takich wartości jak zdrowie i życie człowieka. Rozdział drugi pracy porusza problematykę psychiatrii kulturowej, która zajmuje się badaniem wpływu czynników kulturowych na zdrowie psychiczne. W tymże rozdziale mowa również o stanie badań nad procesem zdrowienia na świecie i w Polsce, zachowaniach wobec problemu, relacjach, postawach w rodzinie i środowisku oraz znaczeniu wsparcia w rodzinie z chorym psychicznie Rozdział trzeci dotyczy chorych psychicznie w procesie zdrowienia w środowisku życia z uwzględnieniem potrzeby bezpieczeństwa, poczucia wsparcia i miejsca osób znaczących, samostanowienia, znaczenia ról społecznych i zawodowych, pozytywnych postaw w rodzinie do wsparcia osobistego i społecznego włącznie. O wzmocnieniu procesu zdrowienia w rodzinie, znaczeniu bliskich i przyjaciół, personelu medycznym oraz instytucjonalnych formach pomocy mowa w czwartym rozdziale dysertacji doktorskiej. Poruszane również kwestie prawne w zakresie zdrowia psychicznego oraz promocja i profilaktyka zdrowia psychicznego w Polsce. Metodologia pracy zawarta jest w rozdziale piątym, natomiast w rozdziale szóstym prezentowane są predyktory procesu zdrowienia osób chorych psychicznie w rodzinie w świetle badań własnych.
Badania własne potwierdziły założenie hipotezy głównej. Dla 39 badanych potrzeba bezpieczeństwa jest fundamentem prawidłowego funkcjonowania człowieka i wywiera wpływ na każdy aspekt jego życia. Akceptacja w rodzinie (37 osób), pozytywne relacje w środowisku życia (36 osób), samoakceptacja (33 osoby), samorealizacja (30 osób), niezależność ekonomiczna to predyktory zdrowienia zdaniem grupy badawczej. Brak informacji, dezinformacja, zwłoka w działaniach są nośnikiem zagrożenia, niepewności i osamotnienia, zdaniem grupy badawczej nie służą zdrowieniu. Okazuje się, że status społeczny nie poprawia poczucia bezpieczeństwa. Jak z dalszych badań wynika nie ma on również wpływu na proces zdrowienia w rodzinie(40)osób. Nie potwierdziła się zatem założona hipoteza. Akceptacja samych siebie w każdej sytuacji (zawsze 36osób), świadczy zdaniem badaczki o dojrzałości grupy badawczej. Ponad 34 ankietowanych zaakceptowało chorobę psychiczną a jest to wymiar rzadko osiągalny w drabinie przystosowania się do niepełnosprawności, szczególnie w chorobie psychicznej, bo trudno zaakceptować chorobę, która nie jest akceptowalne w społeczeństwie, która jest powodem dyskryminacji i stygmatyzacji. Niezmiernie istotnym w procesie zdrowienia jest samostanowienie o sobie, bycie ważnym ogniwem w społeczności rodzinnej i lokalnej. Deklaracje badanych szczególnie w zakresie samostanowienia i samorealizacji ujawniły prawdziwe oblicze. Dokonując analizy zmiennych w zakresie samostanowienia, należy jednoznacznie stwierdzić, że przeprowadzone badania potwierdzają duży dystans do osób z zaburzeniami psychicznymi. Osoby te czują się niedowartościowane, ogranicza im się możliwość rozwoju, pracy. Sam fakt podejmowania decyzji, dokonywania wyboru, nie ma mocy sprawczej. Mimo iż prawnie nie są ubezwłasnowolnieni, to jednak społecznie są „niewidzialni”. Większość grupy badawczej ma status rencisty (21 osób). Aktywnych zawodowo jest tylko 12 osób z grupy badawczej. Status bezrobotnego posiada 16 osób. Dane dotyczące statusu społecznego są niepokojące. . W życiu chorego psychicznie rodzina odgrywa kluczową rolę i stanowi nieocenione wsparcie. Zasoby rodziny są cenne, bo stanowią naturalne warunki życia, w których ten młody człowiek wzrastał, rozwijał się. Analogicznie to środowisko życia powinno być najlepszą bazą do zdrowienia. Jak z badań własnych wynika dużą wagę w powrocie do dobrostanu psychicznego ma: akceptacja osoby chorej psychicznie w rodzinie niezależnie od jego stanu zdrowia (32), bezwarunkowa miłość(21), niegasnącą wiarę, nadzieję w możliwości(14). Dla 37 ankietowanych powrót do zdrowia jest szybszy jak jest pełna rodzina (ojciec, matka, rodzeństwo).
Kolejnym omawianym zjawiskiem badawczym jest wypalenie się sił, które jest ściśle powiązane z długoterminową opieką nad członkiem rodziny z chorobą psychiczną. Rodziny, ale i sami chorzy są bezsilni wobec choroby psychicznej. Utrzymująca się niemoc sprawcza może doprowadzić do bezradności w walce o zdrowie członka rodziny. Ponad 17 badanych grupie wsparcia potwierdziło założoną hipotezę, że bezsilność i niemoc sprawcza towarzyszy walce o zdrowie chorego psychicznie w rodzinie. Natomiast tylko 5 rodzin odczuwa wypalenie się sił w związku z opieką nad chorym psychicznie. Innych(5rodzin) problem nie dotyczy bądź wykazują zrozumienie do zaistniałej sytuacji. Kolejne badania wykazały, że 10 rodzin cierpi razem chorym a ponad 14 miota się w bezsilności. Tylko 2 rodziny otrzymały pomoc duszpasterską, natomiast dla 19 uczestniczących w badaniu, nikt nie zaproponował wsparcia. Środowisko życia jest dla chorego psychicznie, naturalnym środowiskiem, które przy współudziale rodziny, grupy terapeutycznej i terapii farmakologicznej, stanowi właściwe miejsce do rekonwalescencji psychicznej i reintegracji społecznej.
