Title: Sprawność fizyczna i jakość życia dzieci i młodzieży z otyłością prostą

Abstract:

Systematyczny wzrost liczby osób z nadwagą i otyłością w ostatnich latach spowodował uznanie tego stanu za przewlekłą, niezakaźną chorobę cywilizacyjną, ogłoszoną w 1997 roku przez WHO światową epidemią. Zjawisko to jest szczególnie groźne u dzieci, ponieważ negatywnie wpływa na ich późniejszą egzystencję w sferze zdrowotnej, psychicznej i społecznej.
Celem pracy była analiza problemu otyłości u dzieci i młodzieży - w zakresie pomiarów sprawności fizycznej i antropometrycznych, oceny jakości życia zależnej od zdrowia oraz wpływu wybranych parametrów klinicznych. Podjęto także próbę poszukiwania zależności pomiędzy ocenianymi 4 grupami problemów.
Grupę badaną stanowiło 123 pacjentów Kliniki Endokrynologii i Diabetologii oraz Rehabilitacji Pediatrycznej IP-CZD w wieku 8-16 lat (64 chłopców - 52% i 59 dziewcząt - 48%) ze stwierdzoną otyłością prostą. Rozpoznanie otyłości stawiano wg standardu CDC z zastosowaniem siatek centylowych z badania OLAF. Ocenysprawności fizycznej dokonano przy pomocy testu EUROFIT z wykorzystaniem 8 prób, jakość życia badano z użyciem kwestionariusza KIDSCREEN-52, zawierającego 10 domen. Skład masy ciała zbadano metodą bioimpedancji elektrycznej analizatorem BC 418 Tanita. Dane kliniczne zbierano za pomocą pogłębionego wywiadu z rodzicami badanych dzieci.
Uzyskane w niniejszej pracy wyniki badań własnych odniesiono do norm populacyjnych. Łączne wyniki testu EUROFIT w grupie badanej (N = 123) były statystycznie istotnie niższe od normy. Wyniki prób równowagi, szybkości ruchów kończyny górnej, skoczności, siły tułowia, siły funkcjonalnej i zwinności (N = 123) były niższe niż norma, wynik gibkości mieścił się w granicach normy, jedynie siła ręki była wyższa od normy. W 4 z 8 prób sprawnościowych dziewczęta osiągnęły wyniki istotnie lepsze od chłopców. Sumaryczny wynik oceny jakości życia (N = 123) był niższy niż norma populacyjna. W ocenie dziewcząt i ich rodziców jakość życia była istotnie niższa niż norma, wynik oceny chłopców i ich rodziców wykazał tendencję do 139 wartości niższych niż norma, bez cech istotności. Dwa najczęściej obniżone obszary HRQOL to zdrowie fizyczne (W1) i postrzeganie samego siebie (W4).
Wyniki badań własnych sugerują potrzebę monitorowania dynamiki zaburzeń sprawności fizycznej dzieci z nadwagą i otyłością. Wyniki oceny jakości życia z kolei wskazują na potrzebę wsparcia psychospołecznego, szczególnie u dziewcząt, u których dochodzi do jej znacznego pogorszenia w wielu domenach. Wykazano istotny związek sprawności fizycznej i jakości życia w całej badanej grupie oraz niezależnie u dziewcząt i chłopców. Zaobserwowano także istotne związkisprawności fizycznej i jakości życia z wynikami antropometrycznymi i w mniejszym stopniu z danymi klinicznymi. Na podkreślenie zasługuje zwłaszcza wpływ otyłości matki w czasie ciąży na czas karmienia piersią, czas trwania otyłości i niższe wartości indeksu zdrowie fizyczne.


The systematic increase in the number of overweight and obese people in recent years has resulted in the recognition of this condition as a chronic non-communicable disease of civilization, declared a global epidemic by the WHO in 1997. This phenomenon is particularly dangerous in children as it negatively affects their future health, mental and social existence. The aim of the study was to analyze the problem of obesity in children and adolescents in terms of physical and anthropometric measurements, evaluation of health-related quality of life and the impact of selected clinical parameters. An attempt was also made to find the relations between the assessed 4 groups of problems. The study group consisted of 123 patients from Endocrinology and Diabetology and Pediatric Rehabilitation Departments, IP-CZD, aged 8-16 years (64 boys, 52% and 59 girls, 48%) with simple obesity diagnosed. The diagnosis of obesity was made according to the CDC (Center for Disease Control) standard using the percentile grids from the OLAF study. Physical fitness was assessed by using 8 steps of EUROFIT test, the quality of life was tested using the KIDSCREEN-52 questionnaire containing 10 domains/parts. The composition of the body mass was examined by the electric bioimpedance method with the BC 418 Tanita analyzer. Clinical data was collected during a detailed interview with the parents of the studied children. The results of own research obtained in this study were compared with population norms. The total results of the EUROFIT test in the study group (N = 123) were statistically significantly lower than the norm. The results of the balance, speed of upper limb movements, jumping, trunk strength, functional strength and agility (N = 123) were lower than normal, flexibility was within normal limits, only hand strength was above normal. In four out of eight fitness tests, girls achieved significantly better results than boys. The total result of the quality of life assessment (N = 123) was lower than the population norm. In the assessment of girls and their parents, the quality of life was significantly lower than the norm, the result of the assessment of boys and their parents showed a tendency to values below the norm, without any features of significance. The two most frequently lowered areas of HRQOL are physical health (W1) and self-perception (W4). The results of own research suggest the need to monitor the dynamics of physical fitness disorders in overweight and obese children. On the other hand, the results of the assessment of the quality of life indicate the need for psychosocial support, especially in girls who significantly deteriorate in many domains. A significant relationship between physical fitness and quality of life was demonstrated in the entire study group and independently in girls and boys. There were also significant associations of physical fitness and quality of life with anthropometric results and, to a lesser extent, with clinical data. Particularly worth mentioning is the influence of maternal obesity during pregnancy on the duration of breastfeeding, duration of obesity and lower values of the physical health index.
